Before Booking:

• Contact your insurance company to confirm that Dr. Carol Doroshow is listed as an in-network provider with your plan
   

• Fill out our New Patient Packet

​

What to Bring to your First Visit? 

       

• Insurance card, first-time payment for our patient portal system, and co-pay if applicable
     

• The completed New Patient Packet

• Immunization records
   

• Contact info of any previous provider(s) and/or copies of pertinent medical records
   

• Current medication and/or supplements bottles to your appointment so we can see the ingredients and dosages
     

Newborn Patients:

 

• Please contact your insurance right away to add your newborn to your insurance policy
   

• Ask the birth providers to fax all birth records and discharge paperwork to our office
   

• Bring any paperwork that the birth providers gave you to your first appointment with us
