First and Foremost - COVID-19 Screening:


We are committed to being a COVID-19 free facility. Because we are seeing patients in-office for well-visits, medication checks, and immunizations, it is essential that we are a safe place. We have many families who have factors that place them at higher risk, and it is essential that we keep them safe.


  • Anyone coming into our office must have zero viral symptoms in the two weeks before the appointment. Also, everyone they have been in contact with (household members, playmates and their household members etc.) must have zero viral symptoms in the two weeks.

  • Everyone above age two inside of the clinic building must be wearing a secure face mask over their mouth and nose at all times. If you do not have your own, we can provide disposable adult and pediatric face masks!

  • We are keeping our front door locked and our lobby is currently closed to waiting. Because of this, if you arrive early, please stay in your vehicle until the appointment time. Please knock on our clinic door at the time of your appointment and we will take everyone's temperature (forehead scanner) prior to entering the clinic.

What to Bring to your First Visit? 


  • Insurance card, first-time payment for our patient portal system, and co-pay if applicable        

  • Please bring medication and/or supplements bottles to your appointment so we can see the dosages    

  • Contact info of any previous provider(s) and/or copies of pertinent medical records         

  • Immunization records

  • Bring completed forms - Please print, fill out, and bring with you:


At your first visit:


  • If your visit is for a newborn, check to be sure your child has been added to your insurance policy to ensure you have coverage. Many insurance companies do not list newborns on the parent’s policy for the first 30 days.

  • We strongly encourage you to contact your insurance company prior to your appointment to confirm that the provider you have chosen is a provider on your plan and that you understand what your benefits are.
