INSURANCE & FEES

Insurance:

 

We do not accept Community Health Plans (CHP), Apple Health, or any Medicaid plans.

We contract with the following insurance companies, but they do not keep us informed of which plans we are considered in or out of network with. It is critical that you check with your insurance company to see if Dr. Carol Doroshow, MD is in-network for your specific plan. We never want to surprise anyone with a bill they were not expecting.

  • Kaiser Options/PPO

  • Liberty Healthshare

  • Lifewise

  • Premera

  • Regence

  • UMR

  • United Healthcare

For all other insurance companies or out-of-network plans with the above insurance companies, we still welcome you, however, we are an out-of-network provider. You will be responsible for all clinic charges and you will need to file a claim with your insurance company for reimbursement.  If you have an insurance question please feel free to call our billers at 206-957-1891.

Proof of Insurance: 

 

We will verify your insurance and you will be asked to show a copy of the card at each visit. If we are unable to verify your insurance you may be required to pay in full at time of service.

 

Co-Pay Policy: 

 

Please be advised that co-pays are due at time of service. We accept cash, checks, Visa, Mastercard and  American Express credit cards.

Newborn Insurance: 

 

Many insurance companies do not list newborns on the parent’s policy for the first 30 days. Be sure to notify your insurance carrier upon birth of your child to ensure you have coverage for your newborn.

Billing: 

 

We are happy to bill the above insurance companies on your behalf. We use a third party billing company named Larsen Billing Services. (Note for return phone calls that they are located in Utah!) You may reach them at 206-957-1891.

 

Services and charges not covered by your insurance policy will be your responsibility and are due within 30 days of the billing date.
